Does anyone know what the smallest overall (small pitchers,
small leaves, compact structure, etc.) Nepenthes species

Undoubtably the smallest of all the Nepenthes has to be the marvellous tiny
N. argentii (Jebb & Cheek)
We have the only material of this species "in cultivation" but
unfortunately it is proving very difficult
to duplicate the conditions for the strange growth habit. You can check it
